Liver Qi Stagnation is one of the most prevalent TCM syndromes of vitiligo, mostly seen in teenagers, young adults, and people enduring long-term emotional stress. Different from vitiligo triggered by low immunity or sun exposure, this type of white lesion is fundamentally driven by blocked qi movement inside the body, with mood disorders as its core trigger.
In traditional Chinese medicine theory, the liver governs the free flow of qi throughout the body. Constant anxiety, suppressed sadness, frequent irritability, work stress, family disputes or long-term emotional repression will directly obstruct liver qi circulation, forming liver qi stagnation.When liver qi cannot circulate smoothly, the movement of blood will also be blocked. The skin fails to receive sufficient qi and blood nourishment, and the generation of melanin will be restrained, eventually forming pale white patches on the skin. Most patients with this type of vitiligo can recall that their white spots first appeared or suddenly spread after a heavy emotional blow or months of persistent stress.
The liver carries out its vital self-repair work between 23:00 and 3 a.m. If patients regularly stay up past midnight, the liver cannot finish its repair cycle. The function of dredging qi will decline sharply, which easily worsens liver qi stagnation, accelerates the expansion of existing vitiligo and even induces new lesions. This is why many office workers and students who often pull all-nighters are diagnosed with this TCM pattern.
Long-term heavy consumption of spicy food, fried greasy dishes, strong alcohol and stimulating drinks will generate excessive internal heat inside the liver. The accumulated liver heat will further bind stagnated liver qi, double-block the circulation of qi and blood, and make white patches more stubborn. In contrast, a bland diet helps soothe the liver and relieve stagnation.
Long-time sedentary work, staying indoors all day and insufficient exercise slow down the overall circulation of qi and blood across the whole body. Stagnated liver qi cannot be dispersed naturally through movement, creating a vicious cycle: blocked qi leads to vitiligo, and limited activity keeps the qi stuck.
From a modern medical combined TCM view, sustained liver qi stagnation caused by bad moods will disrupt the body’s immune regulation. Abnormal immune cells will mistakenly attack healthy melanocytes in the skin, halting melanin production. This explains why liver qi stagnation vitiligo often progresses faster when patients are under heavy mental burden.
